We require documentation of your current status at an accredited institution. Acceptable forms of documentation include a current student or faculty ID card, letter of enrollment or statement of employment from the institution. You can fax the documentation to 503-214-8581 or attach a pdf or other file format to an email and send to sales@medicineworks.com.
